Sports Physical for Tryouts

Physicals are due prior to the start of clinics on 4/17/2023. If your cheerleader does not have their sports physical, they will NOT be able to participate in clinics and tryouts. If you are unable to obtain your sports physical prior to close of registration, click on documents and download "Physical to be Provided". Upload that form in the place of your physical to complete the registration. Make certain to provide EJC with a copy of a completed physical form prior to 4/17/2023.
